Graduate Student Profile Form
All students need to complete this form at the beginning of the first semester.
Advisor Form
Mandatory for all MS/Thesis and PhD students. Complete during the first semester.
Request for Transfer of Credits
Complete during the first semester.
Directed Individual Study
May be repeated to a maximum of six (6) semester hours. Up to three (3) credits can be used for the Course Work Requirement of the program.
FSU Graduate Add/Drop Form
Add/Drop form for FSU graduate students. Form can only be used for FAMU students during adjustment period.
Grad Course Request for Undergraduates Form
Form requesting to take graduate course as undergraduate student.
TA/Grader Application Form
Complete approximately 1 month before the beginning of the semester.
Supervisory Committee Form
Mandatory for all MS/Thesis and PhD students. Complete by the end of the second semester.
Graduate Assistant Performance Evaluation Form
Mandatory for all the TAs, Graders, and MS students who are receiving RA.
Change of Advisor Form
If you use this form you also need to form a new Supervisory Committee using form.
Exit Interview Form
This exit interview will be used for improving our graduate curriculum and courses.

MS Plan of Study Form
Mandatory for all MS students. Complete or update this form regularly starting with the first semester.
M.S. Thesis Defense Application/Approval Form
MS Thesis students only. To be completed before and after the thesis defense.
MS Student's Annual Evaluation Form
MS Non-thesis students only. To be completed before and after the exam.
M.S. Degree Graduation Checklist
Mandatory for all MS students. To be completed during the final semester.
M.S. Non-Thesis Comprehensive Examination Application/Approval Form
MS Non-thesis students only. To be completed before and after the exam.
Ph.D. Forms
PhD Plan of Study Form
Complete & update this form regularly (starting with the first semester).
Prospectus Examination Application/Approval Form
Complete by the end of the third or fourth semester.

Complete during the first semester.
PhD Preliminary Examination Report Form
Complete during the Preliminary Examination.

Complete during the final semester.
PhD Presentation & Defense Application/Approval Form
Complete during the final semester.
Doctoral Student Annual Evaluation Form
Complete during the annual evaluation, which takes place in February each year.
